Center of H. E. A. R. T (Holistic Empowerment Assessment and Recovery Therapy) a community based mental health agency dedicated to supporting individuals and families impacted by incarceration, substance use, and trauma. Our work centers on healing, stability, and long term success for people returning home from incarceration and for the families who walk that journey alongside them.
We understand that reentry is more than release it is a critical transition that requires structure, support, and individualized care. Our services are designed to address the emotional, behavioral, and practical challenges that arise during this transition, while restoring family connections and strengthening community ties
In 2023, our agency was awarded a contract with the Pennsylvania Department of Corrections to provide no cost counseling services to individuals on state probation and parole. Through this contract, we deliver individual therapy, group therapy, and alcohol and other drug (AOD) treatment, supporting successful community reintegration, compliance with supervision requirements, and long term stability through evidence based, trauma informed care.
Our Intensive Outpatient Program (IOP) is DDAP licensed and provides structured, evidence based treatment for individuals with substance use and co occurring mental health disorders. The program offers comprehensive group and individual counseling, relapse prevention, and recovery focused skill building while allowing participants to remain engaged in work, family, and community responsibilities.
Our services help families navigate reunification, address the emotional impact of separation, and develop healthy coping and support strategies that foster long term stability and resilience. We support families impacted by justice involvement or substance use by providing trauma informed counseling that strengthens communication, rebuilds trust, and promotes healing for the entire family system.
We offer telehealth Intensive Outpatient Program (IOP) and therapy services to residents of Pennsylvania, Delaware, and New Jersey, increasing access to high quality, trauma informed mental health and substance use treatment regardless of location. Our secure virtual platform allows individuals and families to receive consistent, structured care while balancing work, supervision requirements, and family responsibilities
